Diagnosis

Diagnosis is the second phase of the nursing process. It involves analyzing data, identifying health problems, risks, and strengths, and formulating diagnostic statements. 

The goal of diagnosis is to identify the client’s actual or potential health needs that can be addressed by nursing interventions.

Diagnistic Process

There are different types of diagnoses, such as:

  1. Actual diagnosis: describes a problem that is present at the time of assessment.

  2. Risk diagnosis: describes a problem that is likely to develop if interventions are not implemented.

  3. Health promotion diagnosis: describes a desire or readiness to enhance well-being.

A diagnostic statement consists of three parts: the problem (nursing diagnosis), the etiology (related factors), and the signs and symptoms (defining characteristics). For example:

  1. Acute pain related to tissue injury as evidenced by verbal report of 8/10 pain, grimacing, guarding, and elevated blood pressure.

  2. Risk for infection related to surgical incision.

  3. Readiness for enhanced nutrition as evidenced by verbalization of interest in healthy eating.
