Diagnostic Evaluation
The diagnosis of peritonitis is based on the following:
- History and physical examination- The patient may present with signs and symptoms such as abdominal pain, tenderness, rigidity, distension, fever, tachycardia, nausea, vomiting, anorexia, and altered bowel sounds. The pain may be diffuse or localized depending on the extent and location of the inflammation. The patient may also have a history of GI disorders, abdominal surgery, trauma, or peritoneal dialysis.
- Laboratory tests- The white blood cell (WBC) count is usually elevated, indicating infection and inflammation. The serum electrolyte levels may be abnormal due to fluid and electrolyte imbalance. The blood urea nitrogen (BUN) and creatinine levels may be increased due to renal impairment. The blood culture may identify the causative organism of the infection.
- Imaging studies- The abdominal X-ray may show air-fluid levels, indicating intestinal obstruction or perforation. The abdominal ultrasound or computed tomography (CT) scan may reveal fluid accumulation, abscess formation, or organ involvement.
- Paracentesis: involves inserting a needle into the peritoneal cavity and withdrawing fluid for analysis. The fluid is examined for appearance, cell count, protein level, and glucose level.
