In this scenario, the client’s symptoms suggest rapid fetal descent, which requires direct assessment of cervical dilation and station rather than evaluation of discharge characteristics.

Fetal heart rate monitoring is essential for detecting distress, especially during contractions. However, the client’s symptoms strongly suggest imminent delivery, which requires confirmation via vaginal examination. While fetal heart rate patterns guide interventions like oxygen or position changes, they do not provide information about cervical dilation or fetal station, which are crucial in this context for safe and timely delivery planning.

Adequate hydration during pregnancy supports plasma volume expansion, amniotic fluid production, and renal perfusion. The recommended daily water intake for pregnant individuals is approximately 2.3 to 3 liters. Consuming 3 liters falls within this range and does not pose a risk for high-risk pregnancy. Folate is essential for neural tube development, and the recommended daily intake during pregnancy is 400 to 600 mcg. Consuming 400 mcg meets the minimum requirement and supports DNA synthesis, cell division, and fetal neural development. Deficiency increases the risk of neural tube defects such as spina bifida, but this intake level is adequate and does not contribute to high-risk pregnancy status.

Iron requirements increase during pregnancy due to expanded maternal blood volume and fetal demands. The recommended daily intake is 27 mg. This finding relates to uterine involution rather than urinary status and does not provide information about voiding effectiveness.

Bathing a newborn immediately is standard for reducing surface contaminants but does not address the systemic nature of herpes simplex virus acquired in utero. HSV transmission occurs transplacentally or during delivery, and the virus resides intracellularly. Bathing does not prevent viral replication or transmission and is not a priority intervention for managing congenital HSV infection.

Breastfeeding is generally safe in HSV-positive mothers unless active herpetic lesions are present on the breast. A temperature of 37.6°C (99.6°F) is within the upper limit of normal postpartum temperatures. Mild elevations in temperature during the first 24 hours postpartum can result from dehydration, exertion during labor, or breast engorgement. However, this temperature alone is not specific or sensitive enough to indicate hypovolemia. It lacks the hemodynamic significance associated with fluid volume deficit, such as hypotension or tachycardia.

A fractured clavicle is more commonly associated with macrosomia, shoulder dystocia, or difficult vaginal deliveries, particularly in term or post-term infants. At 35 weeks gestation, the fetus is typically smaller and less likely to experience traumatic injury during delivery. Additionally, cesarean delivery is more common in PPROM, which further reduces the risk of mechanical injury such as clavicle fracture. Therefore, this is not a typical complication of PPROM.

Polycythemia in newborns is often associated with intrauterine hypoxia, delayed cord clamping, or maternal diabetes. It is not a direct complication of PPROM. Frequent showering is discouraged in traditional postpartum practices influenced by yin-yang theory. Water exposure, especially cold water, is believed to introduce cold into the body, exacerbating the yin state and leading to long-term health issues like joint pain or fatigue. Instead, hygiene is maintained through sponge baths or dry methods, and warmth is prioritized to restore yang energy and promote recovery.

Applying ice packs introduces cold to the perineal area, which contradicts traditional postpartum beliefs that emphasize warmth to restore balance. While ice packs are medically used to reduce swelling and pain, they may be culturally unacceptable to clients who believe cold exposure can lead to chronic illness. Culturally sensitive care requires adapting interventions to align with the client’s health beliefs when safe and feasible.

The critical congenital heart disease (CCHD) screening is typically performed after 24 hours of life to reduce false positives due to transitional circulation. Screening before 24 hours may yield inaccurate results because of the persistence of fetal shunts like the ductus arteriosus. Therefore, performing the test between 6 and 12 hours is premature and not aligned with evidence-based guidelines for optimal screening accuracy.

CCHD screening is conducted using pulse oximetry, not blood sampling. The test involves placing sensors on the newborn’s right hand (pre-ductal) and one foot (post-ductal) to measure oxygen saturation. Blood sampling is not required and would not provide the necessary data on peripheral oxygenation differences, which are critical for detecting ductal-dependent congenital heart defects.

Results from CCHD screening are available immediately after pulse oximetry readings are obtained. The test is non-invasive and provides real-time oxygen saturation data.
