The fundus would feel boggy or soft, not firm, as the uterine muscles relax and allow blood vessels at the placental site to bleed freely, which contradicts the presented clinical scenario of a firm fundus. The expected blood loss is often profuse and gushing, not a steady trickle.

A perineal hematoma is a collection of blood typically deep in the perineal tissue, often resulting from trauma during delivery. Crying is an expressive behavior and communication method for the newborn, signaling needs like hunger or discomfort, but it is not classified as a primitive or protective reflex. Primitive reflexes are involuntary, automatic motor responses integrated by the central nervous system that typically disappear as the cerebral cortex matures.

Talking is a complex, acquired developmental milestone involving sophisticated cognitive, motor, and linguistic skills. It requires extensive learning and maturation of the cerebral cortex, distinguishing it as a learned behavior, not an innate, involuntary, and transient newborn reflex present from birth.

Walking, or ambulation, is a major gross motor developmental milestone achieved typically between 9 and 18 months of age, requiring significant muscle strength, coordination, and cerebral maturation. While the stepping (or walking) reflex is present at birth, voluntary walking is a learned skill, not a transient newborn reflex.

Administering oxytocic medication is often a subsequent intervention after fundal massage proves insufficient or following a primary assessment confirming uterine atony. This action is not the initial, immediate, and non-pharmacological step to diagnose the cause of bleeding and attempt to control it, which is the fundal check.

While fluid replacement is essential for hypovolemia resulting from hemorrhage, increasing fluid intake is a supportive measure and not the primary, initial action to stop the source of the excessive bleeding. The first priority is to identify and address the cause of the hemorrhage, which starts with assessing the uterine tone.

A distended bladder can displace the uterus and inhibit its contraction, contributing to atony and bleeding. Although having the client urinate (or catheterizing) is an important intervention, the immediate priority for profuse bleeding is to first assess the fundus to rule out and/or treat atony via massage.

Maintaining a client with postpartum psychosis on strict bed rest is contraindicated. While safety is paramount, strict bed rest can exacerbate feelings of isolation, contribute to lethargy, and may not directly address the acute safety and psychological needs, including the potential for self-harm or harm to the infant. Physical activity, when safe, is often encouraged.

Restricting visitation to only the partner can increase the client's social isolation and deprive her of a supportive network, which is vital for recovery. While visitation may need structure, a complete restriction to one person is generally not therapeutic and would not be the standard initial or primary intervention for a psychiatric illness.

While monitoring toileting is essential in contexts where clients may attempt concealed self-harm (like hoarding medications or attempting self-injury), the primary and most acute safety concern in postpartum psychosis is the potential for infant harm, which is a more critical and universal priority than monitoring toileting.

While prolonged or over-stimulated labor can contribute to myometrial fatigue and subsequent atony, the mechanical failure of the uterus to contract effectively (atony) is the direct, primary cause in most cases.

Uterine inversion, a rare but life-threatening complication, occurs when the uterus turns partially or completely inside out. This profound anatomical displacement and associated uterine trauma cause immediate and massive hemorrhage, but its low incidence rate means it is not the most common cause of postpartum hemorrhage.

Cervical or vaginal lacerations can cause significant PPH, especially if associated with rapid delivery or instrumental assistance. This trauma results in bleeding from torn local vessels. While important, lacerations are less frequent than uterine atony as the overall primary cause of PPH, which generally accounts for about 70-80.
